It happened with a colleague who worked as an in-house contact. She was at a meeting with the client's marketing manager.
Manager: How's the YYY Workshop logo development going?
She: Actually, we just finished it. Take a look.
Manager (shouting): It's completely WRONG! You made a logo for YYY Workshop. I wanted a logo for the ZZZ Workshop! How can you make a basic mistake like that?
Manager: Well, I'm sorry, but on your briefing document it says "YYY Workshop". Here, I have a copy of it.
